Website privacy update

Updated November 1, 2026. This notice describes the website privacy controls on apndfw.com. It supplements our existing privacy policy and health information notice; it does not replace the clinical notice or authorize disclosure of health information.

Optional measurement is off

Our website currently disables its Google Analytics, Google Tag Manager and advertising tags. Accepting an optional preference does not switch those services on. We do not use our website measurement component for heatmaps, session recordings or capturing form answers. If we introduce optional measurement, we will update this notice and request a new choice before it starts.

Your choices

Use the Privacy preferences button to review or change your selection. Declining optional measurement does not prevent you from contacting us or requesting services. Your browser preference is separate from consent for treatment, marketing communications or disclosure of health information. Browsing this website is not consent to optional tracking.

Forms, maps and enquiries

The contact-page form loads automatically when you visit that page. Callback and insurance forms load when you open their popup, without requiring acceptance of optional analytics. Opening a form connects your browser to Typeform or Jotform; submitting it sends the information you provide through our enquiry process. The website privacy control does not read your answers. Optional analytics permission is not required to use a form.

The Google map loads only when you choose to open it. This connects your browser to Google. Form and map providers receive connection information needed to serve their content and may use their own storage. Loading an embed does not grant optional analytics permission on our website.

Operating the website

The website and its service providers process connection information, such as IP addresses and requested resources, to deliver pages and protect the service. Turning off optional measurement does not make website use anonymous or prevent this operational processing. The preference control itself does not transmit your choice to a consent server in this release.
